1. 2. Time Limits On Application – This application is valid for only thirty (30) days. If you have not been employed within thirty (30) days of your application, you must re-apply in writing in order to receive further consideration.
3. Completion Of Employment Application – In order to be considered for employment, applicant must complete Parts II and III.
4. The applicant’s signature on Part III agrees to the following:
Consent to Physical Examinations – The applicant consents to take any physical examinations, including but not limited to blood, urine, breath or other examinations or test for alcohol, drugs, or other substance use, that may be requested by Jamison Door Company: (1) in connection with the processing of the Employment Application; and (2) if employment is offered to and accepted by applicant, during the course of applicant’s employment. By completing and signing Part III of this Employment Application, the applicant also understands and agrees that the results of such examinations may be retained by and are the exclusive property of Jamison Door Company. Jamison Door Company assumes no responsibility for advising applicant of the results. All such examinations will be administered by a health care professional designated by Jamison Door Company.
False, Misleading or Omitted Information – Applicant understands and agrees that any false answers or statements or any misleading omissions made by applicant on this Employment Application or in responding to Jamison Door Company’s requests for information in connection with the processing of this Employment Application, including but not limited to false answers or statements or misleading omissions made during interviews or any physical examination, are sufficient grounds for applicant’s rejection for employment or for immediate discharge during the course of employment.
Scope of Employment – Applicant understands and agrees that any employment which might be offered to applicant by Jamison Door Company (other than employment in a position covered by the Agreement between Jamison Door Company and the United Brotherhood of Carpenters and Joiners of America) is "at will" and of indefinite duration; that either applicant or Jamison Door Company can terminate that employment at any time with or without notice for any or no reason; that no agreement to the contrary will be recognized by Jamison Door Company unless made in writing and signed by the C.E.O. of Jamison Door Company and that none of Jamison Door Company’s practices or policies are to be construed as imposing any binding obligations on Jamison Door Company and that they are subject to change or deletion at any time. The applicant understands that, although there is an initial period of the first 90 days of employment, subject to extension at Jamison Door Company’s discretion, during which the applicant will be considered to be in provisional or probationary status and will not be eligible to earn or use certain benefits available to other employees, the applicant’s successful completion of that period will not change the applicant’s status as an at-will employee.
Applicant understands and agrees that any employment which might be offered to applicant by Jamison Door Company in a position covered by the Agreement between Jamison Door Company and the United Brotherhood of Carpenters and Joiners of America will be governed by the terms and conditions of that agreement, where applicable.
Photographs – Applicant understands and agrees that Jamison Door Company may photograph applicant and use such material for security purposes and in Jamison Publications.
Immigration Law – The applicant understands that, as a condition of any offer of employment, the applicant is required by federal law to produce documentary evidence of identity and authorization to work in the United States, and to certify that the applicant is authorized to work in the United States.
Work Permit – The applicant understands that if the applicant is under 18 years of age, the applicant may be required to obtain a valid work permit.
